Learning Objectives
By the end of this activity, participants will be able to describe effective strategies for integrating Community Health Workers, navigators, and other non-licensed personnel into care teams to improve patient engagement and outcomes. Participants will also be able to explain approaches for engaging specialists in value-based care models and aligning their role with population health goals. Additionally, learners will be able to identify how population health teams can use data and care coordination strategies to influence quality outcomes and reduce total cost of care, and apply these concepts to enhance performance within their own organizations.
