The major focus is to advance participants’ understanding of advance care planning and the role of POLST and the development of skills to facilitate goals of care discussion for individuals with advanced illness, serious health conditions, medical frailty or those of advanced age and wishing to define their preferences for care.

Target Audience

Health care professionals, including physicians, physician assistants, CRNPs, nurses, social workers, chaplains and others, who care for individuals approaching the end of life and wish to enhance their ability to engage in effective goals of care conversations with patients, families and surrogates.

Learning Objectives

Participants will:

  • Be able to define the role of Advance Directives and POLST in Advance Care Planning;
  • Have sufficient knowledge of tools and resources to serve as a POLST resource within their institutions and communities;
  • Have a high level of understanding of the importance of the POLST conversation;
  • Demonstrate the ability to facilitate a POLST discussion and appropriately complete a POLST form.

To assist attendees in developing a high level of understanding of the importance of the POLST conversation Present “Conversation Power Point” “Demonstration of a POLST Conversation” (It is recommended that as program is beginning, three attendees be selected or volunteer in advance to read the parts of the resident, staff person and daughter.) Practicum Develop the ability to facilitate a POLST discussion and appropriately complete a POLST form Present “Video Negative POLST” Role Plays – Attendees divided into small groups serve roles as defined on role play handouts Group Activity “Carousel Cases” Power Point Instructors may use all or some of the cases, depending on time.
