Participants in this session will be briefed on the results from the midyear survey and hear a presentation from a PCMH on how they have implemented and sustained SDOH screening, documentation, and community health worker follow-up services, including information on workflows, effective role assignments, and provider training and engagement. Session will also include facilitated peer-to-peer discussion.
