COE PA Navigate - JHF
Participants will learn about the statewide system for Social Determinants of Health / Health Related Social Needs referrals, including how to access and use PA Navigate to identify community organizations for referrals, how to connect to community organizations, and engage participants in sharing experiences and challenges with the referral process and outcomes.
