PA Navigate is a secure website that helps Pennsylvanians connect with community services they need and keeps referrers in the loop. Help is available for needs such as food security, transportation, housing, and more. This session will provide healthcare teams with an introduction to the system and resources available and instruction on using the system to make referrals for patients.
