PCMH 2025 Blood Pressure Control and Asthma Medication Ratio Sprint - Session #1
Members of the healthcare team will develop a deeper understanding of the context and quality improvement framework for the 2025 Blood Pressure Control & Athsma Medication Ration sprint, including HEDIS measures and a Hypertension Change Package. Particpants will also engage in facilitated peer-to-peer learning around each PCMH's priority interventions and quality improvement work.
Agenda:
1:00 p.m. to 1:10 p.m. – Welcome & Presentation of the overall plan for the 2025 sprint, including goals, the description of the measure, and the quality improvement framework
- Share context for the measures.
- Describe HEDIS measures, Controlling High Blood Pressure and Asthma Medication Ratio
- Controlling High Blood Pressure assesses the percentage of members ages 18− 85 years who had a diagnosis of hypertension (HTN) and whose blood pressure was adequately controlled (<140/90 mm Hg) during the measurement year
- Asthma Medication Ratio assesses the percentage of members ages 5−64 years who were identified as having persistent asthma and had a ratio of controller medications to total asthma medications of 0.50 or greater during the measurement year.
