PCMH 2025 Social Determinants of Health (SDOH) Interventions Sprint - Session #2 - JHF
Members of the healthcare team will learn about developing relationships and referral processes with CBOs, including how to use technology such as PA Navigate (Find Help) to support processes to connect patients to SDOH-related services. Technology tools, like PA Navigate / Find Help, and workflows showing how referrals from healthcare providers are received and how the “referral loop” is then closed will be shared. Participants will have an opportunity to learn from peers about challenges and successes in creating effective referral relationships with CBOs.
