Learning Objectives

Upon completion of the activity, clinicians will be more confident about having Goals of Care discussions with patients/families. 
Clinicians will know what elements are essential to a thorough Goals of Care discussion and be able to implement strategies for having Goals of Care conversations with patient/families. 
Clinicians will know where to document Goals of Care conversations in patient charts and understand why it is important to document Goals of Care discussions in a standardized way. 
All learners will understand the impact of early and ongoing Goals of Care discussions on alignment of care, quality of patient care, and patient quality of life. 
Additionally, all learners will have an understanding of the variety of services Palliative Care can provide for the patient/family when a Palliative Care consult is placed and also, will understand the difference between Palliative Care and Hospice Care. After completing this activity, all learners will understand that consulting Hospice earlier, rather than later, allows the patient to receive the full benefit of Hospice care. 
Clinicians will place Hospice referrals/consults as early as is appropriate for patients, based on Goals of Care discussions. 
Clinicians will understand what services Hospice can provide to the patient and family, in addition to when those services can be provided and for how long. Finally, clinicians will be able to place a referral for outpatient hospice services or, a consult for inpatient hospice services, when appropriate.
