PCMH 2026 Behavioral Health Integration Sprint - Session #3 - JHF

Participants will explore strategies for integrating behavioral health services within primary care and discuss common challenges encountered during quality improvement efforts across PCMH practices. They will also share lessons learned from their first Plan-Do-Study-Act (PDSA) cycle, examine preliminary results, and identify approaches for refining interventions and developing goals for the next phase of improvement work.

9:15 a.m. to 9:45 p.m. PCMH Sharing of Challenges and Potential Support

  • Review pre-survey results on challenges and potential PCMH Learning Network support
  • Discussion

9:45 p.m. – 10:15am PCMH Panel Discussion: Moving from PDSA Cycle 1 to 2

  • PCMH Representatives from: Berks Community Health Center
