Participants will examine how peers are applying quality improvement methods to strengthen comprehensive diabetes care within primary care settings. Through presentations, peer discussion, and shared reflection on the transition from the first to second Plan-Do-Study-Act (PDSA) cycle, participants will gain insight into how interprofessional healthcare teams are assessing early findings, identifying implementation challenges, refining goals, and adapting interventions to improve care delivery. The session will support collaboration across disciplines by highlighting practical approaches to quality improvement planning, shared problem-solving, and ongoing evaluation within primary care practice.
