Participants will explore strategies to support successful transitions from pediatric to adult care and learn how pediatric nursing care teams can use practical tools to clarify roles and develop standard work for case managers. They will also discuss progress on quality improvement projects, share lessons learned from their first plan-do-study-act (PDSA) cycle, and identify approaches for implementing and refining interventions during the second PDSA sprint cycle.
