ForwardPCMH PNC Learning Session - PCCRCs - October 16, 2024
Pediatric Complex Care Resource Centers (PCCRCs) were created to provide education, training, and non-medical support to caregiver teams to assist with building relationships and co-navigating the various systems and resources that are available to support them. Session participants will learn about the goals and services of the PCCRCs and how to effectively interface and collaborate with PCCRCs to help with effective care delivery to pediatric patients with complex care needs.
